Description
Rotating through a posterior view of the fibula, the sequence settles on the distal lateral malleolus as it projects inferiorly and slightly posterior to the tibial plafond, framing the lateral aspect of the talocrural (ankle) mortise. Along the posteromedial surface of the malleolus, the malleolar groove (sulcus malleolaris) comes into profile, running longitudinally toward the fibular apex. Subtle contour changes distinguish the articular facet for the talus on the medial side from the roughened attachment areas for the anterior and posterior talofibular ligaments and the calcaneofibular ligament laterally. Clinical relevance sits in that groove. It guides the fibularis (peroneus) longus and brevis tendons behind the lateral malleolus, where tenosynovitis, tendon subluxation after superior fibular retinaculum injury, and posterolateral ankle pain syndromes are localized.
