Description
Lateral malleolus anatomy is presented from a medial perspective, centering on the malleolar fossa on the distal fibula. The concavity lies on the medial surface of the lateral malleolus, just posterior to the smooth articular facet that faces medially to meet the lateral surface of the talus within the ankle mortise. Superior to the fossa, the fibular shaft narrows into the distal metaphysis, while inferiorly the malleolar tip projects distally beyond the level of the tibial plafond. Subtle ridges around the depression help orient anterior versus posterior margins of the malleolus in this fixed view. The malleolar fossa matters because it marks the attachment region for the posterior talofibular ligament, part of the lateral collateral ligament complex that stabilizes the talus against posterior translation and contributes to restraint in dorsiflexion, so its cortical contour is scrutinized in avulsion patterns and in higher-grade lateral ankle sprains.
