Description
Seen from the posterior aspect of the male lower limb, the femoral artery continues through the adductor hiatus to become the popliteal artery in the popliteal fossa, then divides into the anterior tibial artery and the tibioperoneal trunk. Distal to that bifurcation, the fibular (peroneal) artery descends in the deep posterior compartment, running inferiorly and laterally along the medial crest of the fibula, posterior to the tibia and close to the interosseous membrane. Red arterial channels track beside blue venae comitantes, with the tibia positioned medially and the fibula laterally as the bony reference points. For clinicians, the fibular artery matters because it is the most protected of the three crural arteries, often preserved in peripheral arterial disease when the anterior or posterior tibial arteries are occluded. Its perforating branch near the distal interosseous membrane contributes to the anastomotic network around the ankle and can maintain dorsalis pedis flow through collateralization, a pattern commonly assessed in CTA, MRA, and catheter angiography. Surgeons also respect this vessel during fibular osteocutaneous free flap harvest and lateral leg approaches, where injury risks flap perfusion or produces troublesome deep compartment bleeding. Deep and unforgiving.
