Description
Running vertically along the posterolateral lumbar spine, the iliocostalis lumborum forms the lateral column of the erector spinae (sacrospinalis) mass, positioned lateral to longissimus thoracis and spanning from the posterior iliac crest and sacrum toward the inferior angles of the ribs. Tendinous slips reach the lowest ribs (classically ribs 6 to 12), sitting superficial to the thoracolumbar fascia and deep to the skin and subcutaneous tissue. Midline, the spinous processes and thoracolumbar vertebral arches anchor the composition, while the ribs flare laterally and the scapulae and proximal humeri frame the upper back. For teaching biomechanics, this posterior view clarifies how iliocostalis lumborum transmits extension and ipsilateral lateral flexion forces from the pelvis to the rib cage, and why rib motion can provoke pain in lumbar paraspinal strain. It also fits clinical anatomy discussions around the erector spinae plane (ESP) block, where local anesthetic is deposited deep to the erector spinae group near the transverse processes to spread cranio-caudally, a relationship easier to explain when you can distinguish the lateral iliocostalis from the more medial erector spinae components. Palpation and trigger point mapping often target this band just lateral to the lumbar spinous processes and medial to the posterior axillary line. A common site of overload.
