Description
Running longitudinally along the posterolateral lumbar spine, the iliocostalis lumborum appears as a segmented column of muscle bellies within the lateral part of the erector spinae (sacrospinalis) mass. Its fibers sit lateral to the longissimus thoracis and medial to the costal angles and lower ribs, with inferior attachment blending into the thoracolumbar fascia near the posterior iliac crest and sacrum. The spinous processes remain centered in the midline, while the rib cage, scapulae, and posterior pelvis provide bony landmarks for orientation. Surface anatomy matters here. Because the iliocostalis lumborum spans from the pelvis toward the inferior ribs, it is a frequent generator of mechanical low back pain when overworked in repetitive extension, sustained forward flexion, or asymmetrical lifting, and its myofascial trigger points often refer pain laterally toward the iliac crest and lower thorax rather than strictly midline. This posterior relationship also explains why the muscle is encountered and sometimes split in paramedian posterior approaches to the thoracolumbar junction, and why edema or fatty atrophy of the erector spinae on MRI can correlate with chronic lumbar spine pathology or postoperative deconditioning. Clear separation of the erector spinae columns supports teaching of lumbar compartmental anatomy and palpation zones. Use this illustration in gross anatomy and kinesiology courses when demonstrating the lateral erector spinae and its relationship to the ribs, vertebral column, and pelvis in a male back.
