Description
Running longitudinally along the posterolateral thoracic wall, the iliocostalis thoracis is isolated beneath the skin as the lateral column of the erector spinae (sacrospinalis) muscle mass. Its fleshy slips arise from the lower ribs and course superiorly to attach segmentally near the angles of the upper ribs, lying lateral to longissimus thoracis and superficial to the posterior ribs and intercostal spaces. Medially, the thoracic spinous processes and laminae define the vertebral midline, while the scapulae sit posterolaterally over the rib cage, framing the superior extent of the muscle. Because iliocostalis thoracis spans multiple costal attachments, this posterior view is useful for teaching how thoracic extension and ipsilateral lateral flexion couple with rib motion during forced expiration and trunk stabilization. Clinically, tender points and strain in the iliocostalis region can mimic costovertebral or costotransverse joint pain, and the relationship of the erector spinae to the thoracic transverse processes underpins ultrasound-guided erector spinae plane blocks used for thoracotomy, rib fracture analgesia, and some breast and spine procedures. The muscle’s lateral position also helps learners distinguish erector spinae pain patterns from more medial multifidus-related facet pain at the thoracolumbar junction.
