Description
Sphenoid bone landmarks dominate the posterior view, with the infratemporal crest running along the greater wing as a sharp ridge separating the temporal surface superiorly from the infratemporal surface inferiorly. Medial to the crest, the greater wing slopes toward the body of the sphenoid, while laterally it continues toward the cranial wall where it contributes to the floor of the middle cranial fossa. Just inferior and medial to this ridge, the region where the foramina of the greater wing (foramen ovale and foramen spinosum) lie is appreciated by its relationship to the crest and the adjoining infratemporal surface. For skull base teaching, the infratemporal crest is a reliable orientation mark because it predicts the transition from the temporal fossa (temporalis muscle origin) to the infratemporal fossa (pterygoid region), a distinction that underpins pathways for mandibular nerve (V3) blocks and surgical corridors to the parapharyngeal space. From this posterior perspective, the crest reads as a palpable edge in the 3D geometry of the greater wing, helping you mentally register where instruments and needles are directed relative to V3 at the foramen ovale and the middle meningeal artery at the foramen spinosum. The same relationships matter in craniofacial trauma, where fractures through the greater wing can threaten these foramina and the adjacent skull base.
