Description
Arising from the thoracic spinal nerves near the intervertebral foramina, the intercostal nerves are traced posteriorly as they pass adjacent to the dorsal root ganglia and then course laterally into each intercostal space. From this posterior perspective, each nerve runs inferior to its corresponding rib along the costal groove, maintaining a segmental pattern from superior to inferior thorax. The thoracic vertebral column sits in the midline with ribs sweeping anterolaterally, while the scapulae overlie the upper ribs as superficial bony landmarks. Segmental symmetry is easy to read. Posterior visualization matters because it clarifies where the intercostal nerves begin relative to the vertebral bodies and costovertebral joints, the region implicated in radicular pain from thoracic disc herniation or foraminal stenosis. It also supports procedural anatomy: during a posterior approach to paravertebral block, anesthetic placed near the intervertebral foramen targets the spinal nerve and proximal intercostal nerve before extensive anterior branching. Small differences in level matter. Confusing T12 (subcostal nerve) with a typical intercostal nerve is a common teaching pitfall.
