Description
Posteriorly, the semimembranosus is presented as the medial hamstring on the back of the male thigh, with a broad, flat proximal tendon arising from the ischial tuberosity deep to the long head of biceps femoris and adjacent to semitendinosus. Its muscle belly descends inferiorly and slightly medially, approaching the posteromedial knee where the distal tendon passes behind the medial femoral condyle toward its tibial insertion on the posterior medial tibial condyle. Lateral to it, the biceps femoris outlines the lateral hamstring mass, while superiorly the gluteus maximus forms the buttock contour; more proximally still, latissimus dorsi and the erector spinae frame the posterior trunk and provide orientation for the full posterior musculature. Clinically, this posterior relationship matters when sorting medial versus lateral hamstring injury patterns and when explaining why semimembranosus strains often present as deep posteromedial thigh pain that can refer toward the popliteal fossa. At the knee, the semimembranosus tendon contributes expansions to the oblique popliteal ligament and posterior capsule, a detail often discussed in posteromedial corner instability and during arthroscopic planning to avoid iatrogenic injury around the posteromedial portal.
