Description
Running longitudinally along the posterior thoracic wall, the spinalis thoracis appears as the medial, slender column of the erector spinae complex immediately adjacent to the thoracic spinous processes. Positioned medial to longissimus thoracis and deep to the superficial fascia, its muscle fibers course superiorly from lower thoracic and upper lumbar spinous processes toward more superior thoracic spinous processes. A posterior, subcutaneous presentation keeps the overlying skin intact while selectively revealing the paraspinal musculature in situ. Clear midline anatomy. Clinically, this medial erector spinae column is a practical landmark when teaching regional back pain patterns and when discussing posterior approaches to the thoracic spine where midline dissection proceeds through skin and thoracolumbar fascia before the erector spinae mass is mobilized laterally. Spinalis thoracis is also the portion most closely related to the interspinous region, so it helps frame conversations about strain from sustained extension, and about why thoracic paraspinal spasm can localize tightly to the midline rather than radiating laterally. For educators, the spatial relationship to the spinous processes makes it an easy reference point for orientation before introducing deeper transversospinalis muscles.
