Description
Running longitudinally along the posterior thoracic spine, the semispinalis thoracis is presented as a deep transversospinalis muscle with fascicles ascending superomedially from thoracic transverse processes toward the spinous processes of upper thoracic and lower cervical vertebrae. Superficial bony landmarks include the spinous processes, posterior ribs and costovertebral region, with the rib cage curving laterally away from the vertebral column and the pelvis forming the inferior base of the trunk. Portions of the erector spinae mass may appear more superficially lateral to the spinous processes, while semispinalis thoracis sits medial and deep, immediately adjacent to the vertebral laminae. For teaching layered back anatomy, a posterior perspective on semispinalis thoracis clarifies the division between the erector spinae (longissimus and iliocostalis groups) and the transversospinales (semispinalis, multifidus, rotatores). Orientation matters: these oblique fascicles explain why the muscle contributes to extension of the thoracic spine and contralateral rotation, and why deep paraspinal atrophy or fatty infiltration can be assessed segmentally on MRI in chronic back pain or after posterior spinal surgery. Distinguishing semispinalis from adjacent dorsi musculature also helps when discussing midline posterior approaches, where retraction and subperiosteal dissection track close to these deep stabilizers. Anatomy faculty can drop this plate directly into thorax, back, or musculoskeletal modules to support dissection lab orientation and the concept of “deep to superficial” paraspinal layers. It also fits well in spine surgery manuals, radiology teaching files that correlate posterior muscular compartments with imaging, and patient education graphics explaining posterior thoracic pain generators.
