Description
Sectioned testis tissue is presented as a quarter cut, exposing the dense, glistening tunica albuginea (capsule) at the periphery and the connective tissue septa radiating inward toward the mediastinum testis. From the inner surface of the capsule, these septa partition the parenchyma into lobuli testis, creating wedge-shaped compartments arranged around the posterior, slightly medial mediastinal region. Deep to the tunica albuginea, the potential location of the tunica vasculosa and subcapsular vascular plexus is implied along the internal aspect of the capsule. Clear boundaries. This is the view you want when teaching why the testis is not a uniform gland but an organ organized by fibrous architecture that guides both ducts and vessels. The septal framework explains the segmental spread of infection and the patterns of hemorrhage seen after blunt scrotal trauma, while disruption of the tunica albuginea defines testicular rupture on ultrasound and during scrotal exploration. Surgeons also use the relationship of the mediastinum to the capsule when planning testis-sparing approaches for small intratesticular masses, since incisions parallel to septa can reduce unnecessary violation of adjacent lobules.
