Description
Right lateral anatomy places the inferior vena cava (IVC) as a vertically oriented venous trunk ascending on the right side of the vertebral column, receiving the hepatic veins just inferior to the diaphragm before entering the right atrium. Superiorly, the caval opening in the central tendon of the diaphragm aligns the IVC with the cavoatrial junction, while the pulmonary trunk and ascending aorta lie anterior and leftward relative to the right atrium. The right coronary artery and cardiac veins course in the atrioventricular (coronary) sulcus on the anterior surface, framing the right-sided great vessel relationships. Clear right-sided topology. This right-sided perspective matters because it clarifies the short intrapericardial segment of the IVC and its intimate relationship to the inferior wall of the right atrium, a frequent target zone for central venous catheter tips and for cannulation during cardiopulmonary bypass. It also supports teaching of venous return from the abdomen, including how hepatic venous drainage enters the IVC immediately before diaphragmatic passage, a relationship exploited in transjugular intrahepatic portosystemic shunt planning and scrutinized when Budd-Chiari syndrome is suspected. In trauma and oncology, the same anatomy guides interpretation of IVC compression or tumor thrombus extension toward the heart.
