Description

Arising from the lumbar plexus (L2 to L4), the femoral (crural) nerve descends through psoas major and emerges at its lateral border, then passes deep to the inguinal ligament to enter the femoral triangle just inferior to the inguinal crease. In the anterior thigh it lies lateral to the femoral artery and outside the femoral sheath, then rapidly divides into anterior and posterior divisions that fan distally between iliacus, pectineus, and the quadriceps femoris group. Cutaneous branches track anteromedially toward the thigh and, via the saphenous nerve, continue distally along the medial leg. Proximal bony landmarks of the lumbar vertebrae, pelvis, and proximal femur frame the nerve’s course. For teaching, this secondary anterior perspective makes the femoral nerve’s relationship to the inguinal ligament and femoral vessels easy to read. That relationship drives key clinical decisions: a femoral nerve block is placed just lateral to the femoral artery at the level of the inguinal crease, while iatrogenic injury or entrapment beneath the inguinal ligament can present with quadriceps weakness, reduced patellar reflex, and sensory loss over the anterior thigh and medial leg.
