Description
Prominent in the anterior groin and proximal thigh, the femoral nerve (crural nerve) descends from the lumbar plexus (L2 to L4) and passes deep to the inguinal ligament into the femoral triangle. Medial to it lies the femoral artery and vein within the femoral sheath, while the nerve remains outside the sheath on the iliopsoas surface, with the psoas major positioned more medially and the iliacus laterally. Inferior to the inguinal ligament, the nerve quickly divides into anterior cutaneous branches and posterior divisions that track toward the quadriceps femoris and the saphenous nerve. Bony landmarks of the pelvis and proximal femur provide orientation. Clinically, this anterior perspective clarifies why femoral nerve block and fascia iliaca compartment block target the nerve just inferior to the inguinal ligament, lateral to the femoral artery, and superficial to iliopsoas. Small errors in needle position matter. The same relationships explain femoral neuropathy after pelvic or hip surgery, iliopsoas hematoma in anticoagulated patients, and traction or compression within the femoral triangle, with downstream weakness of knee extension (quadriceps) and reduced patellar reflex alongside sensory loss over the anterior thigh and medial leg via the saphenous distribution.
