Description
Prominent mucosal erythema and wall thickening involve the distal esophagus as it descends inferiorly to meet the gastric cardia at the gastroesophageal junction. The inflamed esophageal lumen sits superior to the stomach, while the stomach’s body expands inferiorly and to the left, its rugae folding along the greater curvature laterally and the lesser curvature medially. Proximally, the esophageal tube aligns with the pharynx (throat) and continues through the diaphragmatic hiatus, emphasizing the continuity of the gullet from pharynx to stomach. Clinical correlation is straightforward: this is the pattern most often discussed in reflux esophagitis, where repeated acid exposure injures squamous mucosa near the Z line and can extend proximally with ongoing gastroesophageal reflux disease. For teaching, the juxtaposition of inflamed esophageal mucosa against intact gastric rugae helps clarify why symptoms such as odynophagia and heartburn localize to the retrosternal tract rather than the epigastrium, and it sets up discussion of complications such as peptic stricture and Barrett esophagus at the distal segment. Endoscopic grading, biopsy targets, and the anatomic relationship to the diaphragmatic crura and a sliding hiatal hernia are easier to explain when the gastroesophageal junction is clearly anchored to the cardia.
