Description
Proximal to distal, the pharynx (throat) transitions into the cervical and thoracic esophagus (gullet), which descends posterior to the trachea and heart before traversing the esophageal hiatus of the diaphragm to meet the stomach at the gastroesophageal junction. Inflamed mucosa tracks along the length of the esophageal tube, extending inferiorly to the cardia, where the lumen opens into the gastric fundus and body with prominent rugae. Distally, the stomach curves toward the pyloric region, while the esophageal wall remains relatively straight and narrow as it approaches the lower esophageal sphincter. The contrast is intentional. Esophagitis is most often encountered in the context of gastroesophageal reflux disease, where acid exposure concentrates at the distal esophagus but can extend proximally when clearance is impaired, as in hiatal hernia or supine reflux. A longitudinally inflamed segment also fits pill esophagitis (classically at areas of physiologic narrowing) and infectious esophagitis in immunocompromised patients, and it sets up the teaching pathway from acute mucosal injury to ulceration, stricture formation, and metaplastic change at the squamocolumnar junction (Barrett esophagus). For clinicians, correlating symptom patterns (odynophagia, dysphagia, heartburn) with the anatomy at the upper and lower esophageal sphincters helps explain why endoscopic findings and biopsy sites are chosen where they are. Gastroenterology and pathology educators will find this well suited for slides on esophagitis grading, endoscopy correlation, and the anatomy of the cardia and rugae for upper GI modules in medical, PA, and nursing curricula.
