Description
Talus anatomy is centered on the lateral tubercle of the posterior process, visualized from the lateral aspect of the ankle and hindfoot skeleton. The lateral tubercle projects posteriorly and slightly laterally from the talar body, forming the lateral boundary of the sulcus for the flexor hallucis longus (FHL) tendon, while the medial tubercle lies on the opposite side of that groove. Superiorly, the trochlear surface (talar dome) rises toward the tibial plafond, and inferiorly the posterior facet for the calcaneus slopes toward the subtalar joint, providing positional context for the tubercle within the hindfoot. Posterior talar anatomy matters in posterior ankle pain, where clinicians must separate a prominent lateral tubercle from an os trigonum, and must also recognize fractures of the posterior process that can mimic a simple ankle sprain on initial examination. The fixed lateral perspective makes the relationship between the lateral tubercle and the FHL groove easy to teach, a helpful detail when discussing FHL tenosynovitis in dancers, posterior ankle impingement in forced plantarflexion, or when planning endoscopic posterior ankle approaches that work close to the tendon sheath and adjacent neurovascular structures.
