Along the posteromedial margin, the medial tubercle appears as a blunt eminence set medial to the lateral tubercle, together framing the groove for the flexor hallucis longus tendon on the inferior aspect of the posterior process. Orientation cues keep the talar trochlea anterior and the posterior process trailing posteriorly, so you can track medial versus lateral as the model subtly reorients. Clinically, this small bony landmark matters because it sits at a crowded intersection of posterior ankle structures. Hypertrophy or fracture fragments at the posteromedial talus can contribute to posterior ankle impingement and can irritate the flexor hallucis longus as it courses behind the ankle, a pattern often discussed in dancers and field athletes with pain on forced plantarflexion.
