Description
Centered in the upper cervical spine, the axis (C2) is presented from a superior perspective, emphasizing the paired superior articular facets that sit posterolateral to the dens and slope to receive the inferior facets of the atlas (C1). Between these facets, the dens rises from the vertebral body, with its anterior articular surface facing the anterior arch of C1 and its posterior articular surface facing the transverse ligament. Lateral to the vertebral foramen, the superior surfaces of the pedicles and the bases of the transverse processes frame the articular region. Clean bony landmarks. Orientation of the superior articular surfaces of C2 matters because it sets the mechanics of the atlantoaxial complex, where most cervical rotation occurs and where instability carries a direct neurologic price. In odontoid fractures and transverse ligament disruption, subtle changes in the relationship between dens and the C2 facets can translate into increased atlanto-dens interval and cord risk at the cervicomedullary junction. This view also supports surgical planning for posterior C1–C2 fixation, where appreciating the facet geometry and the corridor toward the pars interarticularis and pedicle helps avoid vertebral artery injury. Use it in gross anatomy and neuroanatomy teaching to anchor discussions of C1–C2 kinematics, dens function, and the articular cartilage footprint on the superior facets of the axis. It also fits well in spine surgery atlases, emergency medicine education on odontoid injury patterns, and radiology correlation pieces when pairing with open-mouth odontoid and sagittal CT reconstructions.
