Description
Viewed from the palmar (anterior) aspect of the ulnar carpus, the pisometacarpal ligament spans distally from the pea shaped pisiform to the base of the fifth metacarpal, crossing superficial to the hamate and the pisohamate region. The pisiform sits proximally and medially on the triquetrum, while the fifth metacarpal base lies distal and lateral, forming the mobile ulnar border of the hand at the carpometacarpal joint. Adjacent osseous landmarks that typically frame this ligament in an anterior hand view include the hook of hamate, the triquetrum deep to the pisiform, and the fourth and fifth metacarpal bases. Articular cartilage is indicated along the carpal, carpometacarpal, metacarpophalangeal, and interphalangeal joint surfaces. Functionally, the pisometacarpal ligament continues the tendon of flexor carpi ulnaris and helps transmit its force to the fifth ray, supporting flexion and ulnar deviation while stabilizing the ulnar side of the transverse carpal region. Clinically, this is the neighborhood of hypothenar pain from pisotriquetral arthritis, traction related irritation at the pisiform, and ulnar nerve symptoms in Guyon canal, where the pisiform and hook of hamate define key borders and guide decompression planning.
