Description
Running transversely across the palmar aspect at the level of the metacarpophalangeal joints, the superficial transverse metacarpal ligament forms a fibrous band linking the volar plates at the heads of the metacarpals. From an anterior (palmar) viewpoint, it sits superficial to the deep transverse metacarpal ligament and lies just distal to the distal palmar crease, bridging between the digital fibrous sheaths at the base of the fingers. The flexor digitorum superficialis and profundus tendons course deep to this plane within their synovial sheaths as they approach the A1 pulleys, while the lumbricals pass from the tendons toward the radial sides of the digits. Neurovascular bundles (proper palmar digital arteries and nerves) run along the lateral margins of each digit, close enough to matter when you are thinking about incisions in the web spaces. This palmar band matters because it helps explain why the metacarpal heads do not splay freely during grip and why the MCP joints behave as a linked unit during flexion, even when individual digits move independently. It is also a useful landmark when teaching the layered anatomy of the palm: skin, palmar fascia, superficial transverse fibers, then the flexor tendon apparatus with its pulleys and volar plates. Small structure, real consequences.
