Description
Positioned on the dorsal aspect of the male hand, the transverse retinacular ligaments span across the metacarpophalangeal region as short transverse bands that tether the extensor mechanism to the underlying joint capsule and volar plate. The carpal bones, metacarpals, and proximal phalanges provide the osseous framework, with interphalangeal and metacarpophalangeal joint lines clearly oriented from proximal to distal. Extensor tendons course distally along the posterior (dorsal) surface of the metacarpals toward the digits, where the retinacular fibers appear as cordlike fascicles running medial to lateral across the dorsal apparatus. Blue accents pick out the tendinous components, separating them visually from adjacent muscle bellies and connective tissue. Dorsal retinacular structures matter because they govern how extensor tendons track during finger flexion and extension, and they help prevent bowstringing and subluxation at the metacarpophalangeal joints. Disruption or laxity in the retinacular system contributes to extensor tendon malalignment seen after trauma, in rheumatoid arthritis with ulnar drift, or following sagittal band injury, and a posterior view makes those relationships easy to teach.
