The distal metatarsals and proximal phalanges align in series, with the ligamentous cords running transversely across the plantar aspect, just proximal to the toe webs and deep to the superficial transverse metatarsal ligament. More proximally, the tarsal bones, including the calcaneus and talus, anchor the longitudinal architecture behind the metatarsal rays. These deep transverse metatarsal ligaments matter because they tether the metatarsal heads into a functional forefoot unit, limiting excessive splay during late stance while allowing controlled transverse arch deformation under load. They form a key anatomic boundary in the third intermetatarsal space where Morton neuroma most often develops, and they are the structure released during operative decompression when conservative management fails; understanding their relationship to the plantar plates and digital neurovascular bundles helps prevent destabilizing the metatarsophalangeal joints. A tight ligament can also contribute to interdigital nerve irritation and plantar forefoot pain in patients with metatarsalgia.
