Seen from the plantar aspect of the male forefoot, the deep transverse metatarsal ligaments span horizontally between adjacent metatarsal heads at the level of the metatarsophalangeal joints. Each band sits distal to the metatarsal necks and proximal to the bases of the proximal phalanges, forming a fibrous bridge across the intermetatarsal spaces. Plantar to these ligaments lie the plantar plates and flexor tendon apparatus, while dorsal to them run the extensor tendons and joint capsules. Clear spacing is implied between the first and second metatarsal heads, where a true deep transverse metatarsal ligament is absent. Plantar visualization matters because these ligaments define the floor of the intermetatarsal spaces and help constrain transverse splay of the metatarsal heads during toe-off. They also form a key relationship for interdigital nerve pathology, since the common plantar digital nerves pass plantar to the ligament before dividing, and compression at this level is central to Morton neuroma between the third and fourth metatarsal heads. When surgeons perform an intermetatarsal decompression or excise a neuroma, knowing the ligament’s distal edge relative to the metatarsal head and plantar plate helps avoid destabilizing the metatarsophalangeal joint or injuring the neurovascular bundle.
