Description
Centered in the anterior pelvis, the sacrum and lower lumbar vertebrae sit superior to the pelvic ring, with the ilia flanking laterally and the pubic bodies meeting medially at the pubic symphysis. Each acetabulum faces anterolaterally and receives the femoral head, with the femoral neck extending distally toward the proximal shaft. Across the front of the coxal joint, the iliofemoral ligament fans from the anterior inferior iliac spine and acetabular rim to the intertrochanteric line, while the pubofemoral ligament courses inferolaterally from the superior pubic ramus to blend with the anterior capsule. Dense fascia and capsular thickening outline a retinacular pattern along the femoral neck. Stability has a price. From an anterior perspective, these capsular ligaments explain why the hip is stiffest in extension and why forced hyperextension can tear the anterior capsule, a mechanism seen in anterior hip dislocation and some athletic subluxations. The iliofemoral ligament is the prime checkrein against extension and external rotation, so its orientation matters during an anterior approach total hip arthroplasty when capsulotomy and subsequent repair influence postoperative stability. Retinacular fibers along the femoral neck also serve as a teaching cue for the course of the lateral epiphyseal vessels, which are jeopardized in displaced femoral neck fractures and contribute to avascular necrosis of the femoral head.
