Description
Presented as an axial cross-section of the vertebral canal, the spinal cord sits centrally within the vertebra, separated from the bony boundary by the meninges and a circumferential ring of cerebrospinal fluid in the subarachnoid space. Within the cord, the butterfly-shaped gray matter forms posterior (dorsal) and anterior (ventral) horns, surrounded by white matter funiculi arranged anteriorly, laterally, and posteriorly. A small, midline central canal lies at the gray commissure, continuous with CSF spaces cranially toward the medulla oblongata. That relationship between CSF, meninges, and neural tissue is the key teaching point. Expansion or contamination of the CSF compartment, as in subarachnoid hemorrhage, meningitis, or intrathecal spread of anesthetic, occurs outside the cord but inside the dura, and this section makes the target space unambiguous. It also supports lesion localization: anterior horn involvement points to lower motor neuron syndromes (for example poliomyelitis or spinal muscular atrophy), while selective posterior column injury aligns with dorsal column deficits seen in vitamin B12 deficiency.
