Description
Anteriorly oriented, the pancreatic plexus is traced as a fine autonomic nerve network accompanying a pancreatic artery as it courses toward the posterior abdominal wall and into the substance of the pancreas. Preganglionic and postganglionic fibers converge around the vessel in a periarterial sheath, with branches extending medially toward the gland and superiorly toward the celiac region. Posterior landmarks logically include the lumbar vertebral bodies and adjacent ribs superiorly, with the pelvis shown inferiorly as a bony frame of reference. Yellow nerve trunks diverge from the paravertebral sympathetic chain and prevertebral plexuses, then follow the artery distally into smaller arterioles. Periarterial nerve plexuses are where visceral pain, autonomic regulation, and surgical planes collide. This view maps the same neurovascular corridor encountered during pancreaticoduodenectomy, distal pancreatectomy, and dissection along the splenic artery, where traction or thermal injury to the pancreatic plexus can contribute to delayed gastric emptying, altered pancreatic exocrine output, and refractory postoperative pain. Chronic pancreatitis and pancreatic carcinoma also recruit and invade these fibers, a well-described substrate for neuropathic abdominal pain and a rationale for targeted celiac plexus block or neurolysis when conservative management fails. Small structure, high consequence. Use this plate for teaching autonomic pathways in gross anatomy and neuroanatomy courses, or for illustrating periarterial dissection concepts in hepatopancreatobiliary surgery texts and interventional pain references discussing celiac plexus and splanchnic nerve procedures.
