Description
Midbrain (mesencephalon) anatomy is centered in this lateral aspect on the lateral groove, a shallow sulcus on the dorsolateral surface that marks the course of the underlying lateral lemniscus as it ascends toward the inferior colliculus. The groove lies posterior to the cerebral peduncle (crus cerebri) and anterolateral to the tectum, placing it between the ventral motor columns and the dorsal auditory midbrain. Superiorly, the groove approaches the level of the superior and inferior colliculi, while inferiorly it trends toward the pontomesencephalic junction where the auditory pathway continues rostrally from the pons. Auditory pathway localization often turns on this landmark. Lesions involving the lateral lemniscus, whether from pontomesencephalic hemorrhage, dorsolateral midbrain infarct in perforator territories, or intrinsic glioma, can produce deficits in sound localization and abnormal auditory brainstem responses, and the lateral groove helps orient those discussions on the external surface. Lighting and surface modeling emphasize the sulcal relief against adjacent convexities of the midbrain, so you can relate an external groove to a deep tract without resorting to a section plane. A practical landmark.
