Description
Nucleus of the posterior commissure lies in the dorsal midbrain at the junction of the pretectum and periaqueductal region, immediately adjacent to the posterior commissure that bridges the midline. From a fixed lateral perspective, the cell group is positioned just superior and posterior to the cerebral aqueduct (aqueduct of Sylvius), embedded within the periaqueductal gray and deep to the tectal surface near the superior colliculus. Its relationship to the dorsal aspect of the brainstem is emphasized, with the aqueduct providing a clean internal landmark for orientation. Small, but specific. Pretectal circuitry matters clinically because lesions in the dorsal midbrain can disturb pupillary light responses and vertical gaze, and the posterior commissure region is a key anatomic corridor for these signs. Pineal region tumors, hemorrhage, or demyelination may compress the posterior commissure and adjacent pretectal nuclei, producing a dorsal midbrain (Parinaud) syndrome, while aqueductal narrowing at this level is a common mechanism for obstructive hydrocephalus. By anchoring the nucleus to the contour of the aqueduct and dorsal midbrain surface, the illustration supports accurate localization for neuroanatomy teaching and for correlating symptoms with a compact set of nuclei in the pretectum.
