Framed from a posterior perspective, the animation centers on the cerebral aqueduct (aqueductus mesencephali, Sylvian aqueduct) as it courses through the midbrain between the third ventricle superiorly and the fourth ventricle inferiorly. The periaqueductal gray forms a tight cuff around the canal, while the tectum (superior and inferior colliculi) lies posterior and the tegmentum lies anterior, defining the aqueduct’s depth within the mesencephalon. As the sequence advances, the aqueduct’s caliber and continuity are emphasized relative to surrounding midbrain landmarks. Small space. Big consequence. Aqueductal patency is the choke point of CSF flow through the ventricular system, and a few millimeters of narrowing can produce obstructive, noncommunicating hydrocephalus with upstream dilation of the lateral and third ventricles. In practice this is where congenital aqueductal stenosis, post-hemorrhagic scarring, or tectal plate lesions become clinically legible, and it is also the anatomic target behind endoscopic third ventriculostomy planning when bypassing the obstructed channel.
