Description
Principal sensory nucleus of the trigeminal nerve occupies the lateral pontine tegmentum at the mid-pons, positioned just medial and slightly dorsal to the trigeminal nerve root entry zone. Its rounded neuronal column lies lateral to the medial lemniscus and paramedian pontine structures, with the adjacent motor nucleus of V situated more medial, and the spinal trigeminal tract and nucleus extending inferiorly into the caudal pons and medulla. From this fixed brainstem perspective, the principal sensory nucleus stands out as a compact gray matter focus within the tegmentum, oriented along the long axis of the pons and aligned with the incoming primary afferents from the semilunar (trigeminal) ganglion. A tight cluster. Functionally, this nucleus is the key relay for discriminative touch, vibration, and conscious proprioception from the face and oral cavity, including dental and periodontal inputs, before second-order neurons project to the contralateral ventral posteromedial (VPM) nucleus via the trigeminothalamic pathways. Small pontine infarcts, demyelinating plaques, or intrinsic pontine lesions that involve the lateral tegmentum can produce ipsilateral loss of fine touch over the face, sometimes alongside trigeminal motor findings if the motor nucleus or exiting fibers are nearby. The emphasis on the nucleus as a discrete, rounded group within the lateral pontine tegmentum supports teaching of brainstem localization, separating the principal sensory nucleus from the spinal trigeminal system that carries pain and temperature.
