Description
Anteriorly, the temporal lobe occupies the inferolateral aspect of the cerebral hemisphere, lying inferior to the lateral sulcus (Sylvian fissure) and anterior to the occipital lobe, with its pole forming the most anterior portion of the middle cranial fossa. The superior temporal gyrus runs parallel to the lateral sulcus, while the middle and inferior temporal gyri sweep anteroposteriorly on the lateral surface toward the temporal pole. Medially, the parahippocampal gyrus and uncus curve around the cerebral peduncle region, positioning the mesial temporal structures inferior and medial to the insula and basal ganglia deep to the opercula. Clear cortical geography. This anterior perspective matters because the temporal pole and mesial temporal cortex are common targets in epilepsy workups and anterior temporal lobectomy planning, where surgeons must keep the relationship of the uncus to the anterior choroidal artery and optic tract in mind to reduce postoperative visual field deficits. The view also helps teach why uncal herniation compresses the ipsilateral oculomotor nerve at the tentorial edge, producing a fixed dilated pupil, and why early medial temporal displacement can threaten the posterior cerebral artery. Spatial orientation drives clinical interpretation.
