Smaller arterioles radiate through the white matter to reach the dorsal horn and intermediate zone, aligning their trajectories with the cord’s medial to lateral organization and the deeper metabolic demands of the gray matter. Nerve rootlets are implied at the periphery, positioned anterolaterally for ventral roots and posterolaterally for dorsal roots in standard anatomical orientation. Clinically, this arrangement frames why anterior spinal artery compromise produces a characteristic anterior cord syndrome, with ischemia of the anterior two-thirds of the cord affecting corticospinal tracts and spinothalamic pathways while often sparing dorsal column modalities.
