Description
Arising most often from the internal pudendal artery within the ischioanal fossa, the inferior rectal artery is traced anteriorly as it runs medially toward the anal canal and inferior rectum. From an anterior perspective in the male perineum, the vessel courses deep to the perineal skin and superficial fascia, skirting the lateral wall of the anal canal adjacent to the external anal sphincter and inferior fibers of levator ani. Small muscular and cutaneous branches radiate toward the perianal region, while accompanying veins mirror the arterial path and drain to the internal pudendal venous system. Fine neural structures from the pudendal nerve and inferior rectal nerves are expected in close proximity. Tight anatomy. This angle matters because the inferior rectal artery sits in the operative corridor for common perineal procedures, and it is a frequent source of bleeding during hemorrhoid surgery, fistulotomy, and drainage of ischioanal abscess. Its relationship to the pudendal canal (Alcock canal), the ischioanal fat, and the external anal sphincter explains why even small arterial branches can retract and be difficult to control when injured. The view also supports teaching of anastomoses between inferior rectal branches and the superior rectal artery, relevant to collateral flow in pelvic vascular disease. Use this illustration in pelvic anatomy teaching blocks, colorectal surgery atlases, and perioperative education materials that discuss perianal bleeding, pudendal neurovascular anatomy, or stepwise approaches to the ischioanal fossa.
