Description
Arising from the medial clavicle and the anterior sternum with slips from the costal cartilages, the pectoralis major spreads laterally in a broad, fan-shaped sheet across the anterior thorax. Its fibers converge toward the proximal humerus, forming a thick tendon that passes anterior to the glenohumeral joint to insert on the lateral lip of the intertubercular sulcus (crest of the greater tubercle). Deep to the muscle, the rib cage and sternum define the anterior chest wall, while the clavicles and scapulae frame the superior and lateral limits of the shoulder girdle. A true anterior perspective clarifies how clavicular and sternocostal heads create differing fiber directions, a point that matters when teaching adduction and internal rotation strength testing or when interpreting deformity after rupture. Pectoralis major tears often occur at the humeral insertion during eccentric loading (bench press), and this view makes the tendon’s lateral convergence and clinical palpation corridor toward the axillary fold easier to explain. It also supports surgical planning concepts for pectoralis major tendon repair and for musculocutaneous flap design, where the muscle’s broad origin and lateral tendon footprint dictate mobilization.
