Description
Running obliquely along the anterolateral neck, the sternocleidomastoideus is rendered bilaterally with clear separation of its sternal head from the medial clavicle and its clavicular head from the superior surface of the clavicle. Each belly ascends superiorly and posteriorly to insert on the mastoid process and adjacent lateral portion of the superior nuchal line, framing the anterior cervical region on either side of the laryngeal midline. Deep relationships are implied by the bony context, with the muscle crossing superficial to the carotid sheath region and lying anterior to the cervical spine, while the clavicles, manubrium, and upper ribs anchor the orientation. Palpation and surface anatomy live or die by this muscle. The anterior perspective clarifies how the two heads create the lesser supraclavicular fossa between them, a corridor used as a landmark for internal jugular venous access and for orienting a cervical plexus block as the superficial branches emerge at the midpoint of the posterior border. Torticollis and neck masses also get easier to teach in this view, since unilateral sternocleidomastoideus shortening rotates the face to the contralateral side with ipsilateral lateral flexion, and enlarged cervical lymph nodes are often described relative to its anterior and posterior borders.
