Centered on the midface in an anterior facial dissection, the levator labii superioris alaeque nasi is identified adjacent to the lateral border of the nose, with fibers running inferiorly from the frontal process of the maxilla toward the upper lip and the alar cartilage. Medial to it, the nasalis spans the nasal dorsum and ala, while laterally the levator labii superioris descends toward the superior lip, blending with the orbicularis oris complex near the modiolus. Superiorly, the inferior fibers of orbicularis oculi frame the orbital aperture, and inferiorly the perioral musculature outlines the oral fissure. Fiber direction is explicit. This anterior perspective matters because it clarifies which muscle you are recruiting when a patient elevates the upper lip versus when they dilate or compress the nostril.
