Prominently featured beneath the superficial facial integument is the levator labii superioris alaeque nasi, a narrow vertical slip running along the lateral aspect of the nose from the frontal process of the maxilla toward the upper lip and the alar cartilage. Medially it hugs the nasal sidewall adjacent to the nasal bones and upper lateral cartilages, while laterally it lies near the levator labii superioris and the more obliquely oriented zygomaticus minor. Inferiorly its fibers blend into the musculature of the upper lip at the level of the philtrum and lateral lip, immediately superficial to the maxillary vestibule. Orientation matters in this region because the levator labii superioris alaeque nasi is one of the primary elevators of the upper lip and a direct contributor to alar flaring, so its vector explains the combined movement seen in expressions of disgust and in some patterns of hyperactive upper lip.
