Presented from the anterior aspect, the right and left lungs fill the thoracic cavity on either side of the mediastinum, with the apex of each lung extending superiorly above the level of the first rib and the diaphragmatic surface curving inferiorly. The right lung is divided into superior, middle, and inferior lobes by the horizontal and oblique fissures, while the left lung shows only superior and inferior lobes separated by an oblique fissure, its cardiac notch indenting the anteromedial border. Diffuse black-brown pigmentation consistent with anthracotic carbon deposition is distributed across the visceral pleura and underlying parenchyma, with irregular pale bands suggesting subpleural scarring, most apparent toward the upper lobes and along fissural surfaces. The central hilar regions lie medial on each side where the main bronchi and pulmonary vessels enter, even if the emphasis remains on the altered lung surface. Carbon pigment in smoker’s lung is more than a cosmetic finding, it correlates with impaired mucociliary clearance and chronic airway inflammation that can progress to chronic bronchitis and centrilobular emphysema, classically with upper lobe predominance. Subpleural fibrosis and focal scarring are common in long-term tobacco exposure and can complicate surgical planes during thoracotomy or video-assisted thoracoscopic surgery, where pleural thickening and adhesions increase air-leak risk after wedge resection. Pathology teaching often pairs this gross appearance with microscopic findings such as anthracosis around respiratory bronchioles and smoke-related dysplasia of bronchial epithelium. It is a warning sign.
