Description
Arising posterior to the manubrium, the right and left brachiocephalic (innominate) veins form from the union of the internal jugular vein and subclavian vein at the venous angle on each side of the lower neck. From an anterior perspective, the left brachiocephalic vein courses obliquely inferomedially across the superior mediastinum toward the midline, while the right brachiocephalic vein runs a shorter, more vertical path to meet it. Their confluence creates the superior vena cava just inferior to the level of the first costal cartilage, deep to the sternum and medial ends of the clavicles. Short segments of the external jugular veins may be suggested superficially, but the emphasis stays on the deep venous trunk. Anterior visualization of the brachiocephalic vein matters because it is the venous gateway for central access from the neck and upper limb, and its relationship to the sternoclavicular joints and first rib guides safe needle trajectory for internal jugular and subclavian catheterization. Left sided central lines traverse the long, transverse left brachiocephalic vein, a common site for catheter tip malposition and thrombosis, and an anatomic reference point when teaching why right sided access is often preferred.
