Description
Prominent in the anterior cervical column, the axis (C2) sits immediately inferior to the atlas (C1) and supports the skull through the atlantoaxial complex. Rising from the superior surface of the C2 vertebral body, the dens (odontoid process) projects superiorly in the midline, positioned posterior to the anterior arch of C1 and anterior to the spinal canal. Lateral to the body, the superior articular facets of C2 face superiorly to meet the inferior facets of C1, while the transverse processes extend laterally toward the course of the vertebral arteries. Intervertebral discs are seen between the lower cervical vertebral bodies, but the C1 to C2 level remains disc-free. A distinctive joint. Attention to the dens is not academic, it is where stability and disaster meet. Fractures of the odontoid (Anderson and D’Alonzo types I to III) and atlantoaxial instability from transverse ligament injury can threaten the cervicomedullary junction, and the anterior perspective clarifies why anterior displacement at C1 to C2 compromises the space available for the spinal cord. This orientation also aligns with the corridor used for anterior odontoid screw fixation, where dens angulation and the relationship to the C2 body guide trajectory selection, and with teaching the rotation mechanics that make the axis the dominant contributor to cervical axial rotation.
