Rotating in an anterior orientation, the humeral shaft is presented with emphasis on the anterolateral surface, the smooth expanse between the anterior border and the lateral border as the contours of the diaphysis come into relief. Proximally, the surface is read in relation to the greater and lesser tubercles and the bicipital groove (intertubercular sulcus) on the anterior aspect, while distally it narrows toward the supracondylar region as the shaft flares toward the lateral epicondyle. Subtle changes in curvature clarify how the anterolateral face transitions into the lateral surface and posterior shaft. Landmarks stay oriented. Clinically, this is the corridor where surface anatomy, fracture patterns, and fixation strategies intersect. Anterolateral midshaft humeral fractures and surgical approaches place the radial nerve at risk as it traverses posteriorly in the radial (spiral) groove and then pierces the lateral intermuscular septum to enter the anterior compartment, a relationship that becomes easier to teach when the bone is seen turning in sequence.
