Description
Arising from the anterior aspect of each external carotid artery, the superior thyroid arteries descend anteroinferiorly toward the superior poles of the right and left thyroid lobes, then fan across the anterolateral glandular surface. A short initial curve is evident near the carotid bifurcation region, with the paired vessels remaining lateral to the midline isthmus as they approach the upper gland. The thyroid itself sits anterior to the trachea, its lobes flanking the airway with the isthmus crossing the midline. Branching caliber decreases as the vessels arborize over the capsule. Surgeons care about this course because the superior thyroid artery is commonly encountered early in thyroidectomy and superior pole dissection, and its relationship to the external branch of the superior laryngeal nerve (EBSLN) dictates where ligation is safest. Tie too high, and voice projection suffers. A clear anterior rendering also reinforces why the superior pole bleeds briskly, and why meticulous control of capsular branches reduces hematoma risk after surgery.
