Suprapatellar Synovial Bursa, Anterior View

Description

Anteriorly, the distal femur and proximal tibia frame a total knee arthroplasty construct, with metallic femoral and tibial components aligned across the joint line and a blue patellar button seated on the anterior surface of the patella. Superior to the patella, the suprapatellar synovial bursa (suprapatellar recess, superior pouch) extends proximally deep to the quadriceps tendon, continuous with the knee joint cavity. The prepatellar bursa lies more superficial, anterior to the patella and patellar tendon, and should be understood as a separate potential space from the suprapatellar pouch. Planes matter here. Clinically, distension of the suprapatellar pouch is one of the clearest surface-correlated signs of a knee effusion, and it is the target space for many ultrasound-guided assessments when the patella is difficult to ballot. In the arthroplasty setting, synovial hypertrophy and recurrent effusions can persist after total knee replacement, and the superior pouch is also where postoperative hemarthrosis and intra-articular infection may first declare themselves as tense suprapatellar fullness. An anterior perspective keeps the relationship between the patella, quadriceps tendon, and superior recess unambiguous when teaching why the suprapatellar recess communicates with the joint while the prepatellar bursa does not. Orthopedic surgery texts and patient-facing arthroplasty education materials often need a clean diagram to explain where fluid collects and where prosthetic components sit relative to the extensor mechanism. Sports medicine and radiology lectures can pair this view with sagittal ultrasound or MRI to orient learners to the suprapatellar recess before discussing aspiration routes and bursal versus intra-articular pathology.
