Suprapatellar Synovial Bursa, Anterior View

Description

Anteriorly, the knee is opened to emphasize the suprapatellar synovial bursa (suprapatellar recess) as a superior extension of the synovial cavity deep to the quadriceps tendon and proximal to the patella. Superiorly it tracks beneath the distal femur, while inferiorly it communicates with the patellofemoral compartment; the bursal wall is formed by synovial membrane reflecting off the anterior distal femur toward the suprapatellar fat pad. Distal femoral and proximal tibial articular surfaces are capped by total knee arthroplasty components, with a tibial polyethylene insert interposed between femoral and tibial metal trays. Clinically, the suprapatellar pouch is the most accessible reservoir for knee joint effusion, and its distension displaces the quadriceps tendon anteriorly, a common physical exam finding in synovitis. After knee replacement, persistent suprapatellar swelling raises a different differential than in the native knee, including reactive synovitis, hemarthrosis, or periprosthetic joint infection, and the recess becomes a target for ultrasound-guided aspiration performed lateral to the quadriceps tendon above the patella. Subtle anatomy matters here.
