Description
Oriented from an inferior (basal) perspective, the cerebral hemispheres frame the midline brainstem, with the optic chiasm and hypothalamic region lying anterior to the midbrain and the pons expanding transversely before tapering into the medulla. Red arterial channels course along the ventral surface and converge around the region classically associated with the Circle of Willis, while blue venous structures track in parallel but remain distinct by color coding. Attention is directed to the ethmoidal artery as it leaves the orbit near the medial wall and passes through the anterior ethmoidal foramen to enter the anterior cranial fossa, then descends through the cribriform plate to supply the superior nasal septum and lateral nasal wall. Small vessel caliber is clear. Seeing the ethmoidal artery in the context of adjacent basal neurovascular anatomy matters when you need to teach where orbital supply communicates with the nasal cavity and anterior cranial fossa. Anterior ethmoidal artery injury is a known cause of brisk anterior epistaxis and can complicate functional endoscopic sinus surgery, ethmoidectomy, or repair of cribriform plate cerebrospinal fluid leaks, where the vessel can retract into the orbit and bleed behind the globe. This angle also reinforces the close relationship to olfactory filaments traversing the cribriform plate and to the anterior cranial fossa floor, a frequent pathway for spread of infection or tumor between sinonasal and intracranial compartments.
