Description
Perpendicular plate of the ethmoid occupies the midline in an inferior perspective, its paper-thin lamina descending from the region of the cribriform plate to form the superior bony component of the nasal septum. Lateral to the plate, the paired ethmoidal labyrinths (lateral masses) frame the septal space, with the attachments of the superior and middle nasal conchae arising from their medial aspects and projecting into the nasal cavity. Inferiorly, the free margin of the perpendicular plate approaches its articulation with the vomer and the septal cartilage, creating a continuous partition between right and left nasal airways. Midline anatomy is the focus. For endoscopic sinonasal work, this viewpoint clarifies how little bone separates the nasal septum from the ethmoidal air cells and skull base, a relationship that becomes unforgiving during septoplasty, functional endoscopic sinus surgery, and ethmoidectomy. Septal deviation, septal spur formation at bony-cartilaginous junctions, and iatrogenic perforation are easiest to teach when the thinness and straight-line orientation of the perpendicular plate are emphasized against the more complex contours of the ethmoidal labyrinth. The inferior orientation also supports correlation with coronal CT anatomy, where the perpendicular plate is traced inferiorly toward the vomer while the ethmoid air cells and conchae sit laterally.
