Description
Perpendicular plate of the ethmoid forms the superior bony portion of the nasal septum, presented here in anterior view as a thin, midline lamina descending inferiorly from the ethmoid. Superiorly, the plate continues toward the crista galli and the region of the cribriform plate, while its inferior margin is the surface that normally articulates with the vomer and septal cartilage (though those elements are not included in this isolated focus). Laterally, the ethmoidal labyrinths sit to either side, framing the plate as the medial wall of the ethmoid complex and orienting the viewer to the nasal cavity midline. A narrow bone. Septal alignment is a daily clinical problem, and the perpendicular plate is one of the structures that bends, fractures, or displaces with nasal trauma, contributing to septal deviation and persistent nasal obstruction. In septoplasty planning, its relationship to the superior septum matters because aggressive instrumentation near the ethmoid roof risks destabilizing attachments near the cribriform region, where iatrogenic cerebrospinal fluid leak becomes a real concern.
