Description
Oriented from an inferior perspective, the cranial base is framed by the ventral surface of the cerebrum, the midline brainstem, and paired venous outflow channels rendered in blue, converging toward the jugular bulbs at the jugular foramina. Superficial cortical veins course over the inferior aspects of the temporal and occipital lobes and feed into larger dural venous sinuses along the posterior cranial fossa, which then funnel laterally and inferiorly into the internal jugular veins. Yellow cranial nerves radiate from the pons and medulla, crossing near venous structures around the petroclival region. Spatially, the jugular bulbs sit inferolateral to the medulla and lateral to the foramen magnum region. Inferior venous anatomy at the skull base is where students often lose the plot. This angle supports teaching the continuity of intracranial dural sinuses with extracranial drainage through the jugular foramen, a point that matters in mastoid and petroclival disease where sigmoid sinus thrombosis can propagate into the jugular bulb and internal jugular vein. Skull base approaches, including retrosigmoid and far-lateral exposures, also hinge on respecting the relationship between lower cranial nerves (IX, X, XI) and the jugular bulb, which can be high-riding and encroach on surgical corridors. Neuroanatomy and head and neck anatomy courses can use this rendering to correlate dural venous sinus pathways with the brainstem and cranial nerve root exit zones in a single field.
